Alberta Health Services School Immunization Program will be at Collingwood School on May 4, 2026 to immunize Grade 1 and Grade 6 students and other eligible students.

Children who are eligible to receive immunizations at school will have received an immunization consent package.

Please review the information, fill out the consent forms, and return these forms to the school as soon as possible (if you still have the package).

If you received a Request for Vaccination Records, please fill out the form and follow the included instructions for providing immunization records to Alberta Health Services. T

he School Immunization Program follows the Alberta Immunization Schedule. If you would like more information about the immunizations that your child has been offered, please visit the website www.ahs.ca/immunize.
